Referral Form

Please be aware that we have a strict policy of returning patients back to you for care once the requested treatment has been carried out. We will also keep you informed and updated during and after the pictures. Please contact the practice for any further information.

N.B. Ref. Orthodontic Referrals

If you answer no to ANY of the below questions, please DO NOT refer to the orthodontist:

Is the patient above 12 years old? Yes/No… if the answer is no, please refer to the hospital if you think early intervention is necessary
Is the oral hygiene good? Yes/No… If the answer is no, do not refer! Patient must only be referred when oral hygiene is good
Is the patient keen for orthodontic treatment and able to cooperate sufficiently? Yes/No
